Flu Vaccine for All Students
If you would like your child to receive the flu vaccine in school on Friday 11 October 2024, please click the blue button below to complete the NHS online consent form. If you would like to decline the vaccine for your child, please complete the online form to provide ‘NO CONSENT’. The deadline for completing the form is 29 September 2024.
Vaccinations are only administered where parent / carer consent has been provided.
