DTP / ACWY vaccinations for Years 9

Friday 14 February 2025 (rescheduled from 3 February):

If you would like your child to receive the DTP / ACWY vaccines in school on the above date, please click the blue button below to complete the NHS online consent form. If you would like to decline the vaccine for your child, please complete the online form to provide ‘NO TO CONSENT’.

The online form will close on 6 February 2025. Paper-based consent forms are also available from Main Reception.

The Secondary School Immunisation & Vaccination Team comes into the Academy throughout the year to provide vaccinations to our students.
